How do I Register?

Registration is as follows:-

Step one:

Fill out the Enrollment Form

 

Step two:

Pay to the ‘GRAPES FOR A GOOD LIFE LTD’ account at any First Citizens Bank Branch, account number 2 2 1 3 6 8 1.

Step three:

• Use your receipt to fill out the information in the Confirm Bank Deposit Form.
• A representative will then call within thirty minutes or less after your payment confirmation to get you started.
• Or you can call 1(868) 280-LIFE after making your payment.
